NOTICE OF PRIVACY PRACTICES ACKNOWLEDGEMENT OF RECEIPT

We welcome you to our practice and look forward to meeting and working with you. We understand the stress that our patients and their families are under during this time and will strive to make your visit as pleasant as possible.

Your physician has referred you to our office for specialized care. If you are unable to keep your appointment, please call our office as soon as possible. When you come to the office, please complete and bring with you the Medical History Form supplied to you by our office. You will also need to bring a list of all medications you are currently taking, all medical insurance information (insurance card, etc.) and all medical records supplied to you by your physician.

Whether your appointment is at our main Asheville office or at one of our four satellite locations, you can find directions here.

Our doctors, as well as our trained oncology nurses, are available during regular office hours, 8:00 AM to 5:00 PM, Monday through Friday for any questions or concerns you may have. You can reach our office by calling the Asheville number 253-4262 or 1-800-506-2550. One of our doctors is on emergency call evenings, weekends and holidays.

When you call our office with medical problems you will initially speak with our triage nurses. You may be refered to one of our nurses.

Our office does file insurance claims for you and we do accept assignment from Medicare and Medicaid. It is also very important that we know if pre-authorization is required by your insurance company. If you have any financial questions or concerns, please contact our billing office.

If, at any time, you have concerns for which you cannot obtain a satisfactory solution, please feel free to speak with me directly. Again, we welcome you to our office. We want your experience in our office to be as pleasant as possible.
